Category 01

Fatigue, low energy, afternoon crashes

Start here

Ferritin

What it is: The storage form of iron in your body.

Why it matters: Low ferritin is one of the most commonly missed drivers of fatigue — and it rarely shows up as “anemia” on a standard CBC. Your iron could look fine while your ferritin is depleted. We see this constantly in women who are told everything looks normal.

What to watch for: “In range” on most lab reports means anything above 12 ng/mL. Functionally, we look for ferritin above 70 — especially in women who are still menstruating. If yours is in the 20s or 30s and you’re exhausted, this is a significant finding.

“Can you add ferritin to my bloodwork?”

Vitamin D (25-OH)

What it is: The storage form of Vitamin D — the most accurate way to assess your actual levels.

Why it matters: Vitamin D plays a direct role in energy regulation, immune function, mood, and sleep quality. Deficiency is extraordinarily common — especially in women who spend most of their day indoors — and it rarely produces obvious symptoms until levels are significantly depleted.

What to watch for: The conventional “sufficient” cutoff is 30 ng/mL. Functionally, we look for 60–80. Supplementing without knowing your baseline is guesswork.

“Can you check my Vitamin D — specifically the 25-OH marker?”

TSH (Thyroid Stimulating Hormone)

What it is: A signal sent from your brain to your thyroid, telling it how hard to work.

Why it matters: TSH is the standard thyroid marker most GPs run. It’s a starting point — but only a starting point. TSH tells you whether your brain thinks there’s a thyroid problem. It doesn’t tell you whether your thyroid is actually converting hormones properly. That’s why if TSH comes back normal and you’re still exhausted, you push to Tier 2.

What to watch for: Conventional normal is 0.4–4.0 mIU/L. Functionally, we prefer to see it between 1.0–2.0. A TSH of 3.8 is “normal” — but it’s also the upper edge of a range where many women feel symptomatic.

CBC with Differential

What it is: A broad look at your red blood cells, white blood cells, and platelets, broken down by type.

Why it matters: Read carefully, the CBC can surface anemia patterns, immune system stress, and B12/folate deficiency — all of which show up as persistent fatigue and poor recovery. Most GPs look at it briefly and move on.

Full Thyroid Panel: Free T3, Free T4, Reverse T3, TPO Antibodies

Why this goes beyond TSH: TSH is one signal. The full picture tells you whether your thyroid is producing enough hormone (Free T4), whether your body is converting it into the active form you can use (Free T3), whether it’s converting it into a form that blocks the active version (Reverse T3), and whether your immune system is quietly attacking your thyroid (TPO Antibodies).

You can have a perfectly normal TSH and still have:

  • Low Free T3 — a conversion problem, often gut-related
  • High Reverse T3 — a stress response where your body is deliberately slowing you down
  • Elevated TPO antibodies — early Hashimoto’s, often undiagnosed for years

This is one of the most common patterns we see in women who’ve been told their thyroid is fine.

Fasting Insulin

What it is: How much insulin your pancreas is producing while you haven’t eaten.

Why it matters for energy: When fasting insulin is elevated, your body is working overtime to manage blood sugar — and that work is exhausting. Energy crashes, difficulty waking up, and that heavy feeling in the afternoon are often insulin-driven, not sleep-driven.

What to watch for: Most labs report normal as anything under 25 mIU/L. Functionally, we prefer to see it under 8.

DHEA-S

What it is: A hormone produced by your adrenal glands — a precursor to both estrogen and testosterone.

Why it matters: DHEA-S is a marker of adrenal reserve — how much capacity your stress response system has left. Low DHEA-S often correlates with burnout, poor recovery, and a flattened stress response. It declines with age but chronic stress accelerates that decline significantly.

Saliva Cortisol Panel (Adrenal Stress Index)

What it is: A 4-point saliva test taken throughout the day — morning, noon, afternoon, and evening — to map your cortisol curve.

Why a blood cortisol test isn’t enough: A single blood draw gives you one number, at one moment, under the stress of a needle. That’s not useful. Your cortisol is supposed to follow a curve — high in the morning, declining through the day, low at night. When that curve is disrupted — flat all day, spiking at night, inverted — it produces a very specific set of symptoms: waking exhausted despite sleeping, crashing at 2pm, wired at 10pm. This is HPA axis dysregulation, and it’s one of the most common and most missed patterns in women with chronic fatigue.

When fatigue has multiple drivers — thyroid conversion issues, low ferritin, insulin dysregulation, depleted adrenals — treating any one of them in isolation produces partial results. Our comprehensive panel reads 40+ markers together, so we can see the full picture and address the actual pattern, not the individual number.

Category 02

Weight gain, puffy or soft midsection, clothes fitting differently

Fasting Glucose

What it is: Your blood sugar level after not eating for at least 8 hours.

Why it matters: Fasting glucose is a snapshot — useful, but incomplete on its own. What it misses is how hard your body is working to keep that number normal. You can have normal fasting glucose and still have significant insulin resistance. This is why it needs to be read alongside fasting insulin.

What to watch for: Conventional normal is under 100 mg/dL. Functionally, we prefer to see it under 90. Anything in the high 90s — especially paired with unexplained weight gain — is worth investigating further.

HbA1c

What it is: A 3-month average of your blood sugar levels.

Why it matters: Unlike fasting glucose, HbA1c shows the trend. It’s one of the best early indicators of metabolic dysfunction — often years before a clinical diagnosis. Weight that accumulates around the midsection despite no dietary changes is a classic sign that something metabolic is shifting.

What to watch for: Conventional normal is under 5.7%. Functionally, we look for under 5.4%. The 5.4–5.7% range isn’t “fine” — it’s a window where intervention changes the trajectory.

TSH

Why it’s here too: An underactive thyroid slows metabolism, drives fluid retention, and is one of the most common contributors to unexplained weight gain in women. Start here, push to the full panel in Tier 2 if it comes back normal.

Fasting Insulin

Why this is the most important metabolic marker most doctors never run: Fasting glucose can look completely normal while fasting insulin is elevated — meaning your body is working overtime to keep blood sugar stable. That’s insulin resistance. It shows up years before glucose becomes a problem, and it’s directly connected to weight gain around the midsection, carbohydrate sensitivity, and energy crashes after meals.

What to watch for: Most labs report normal as anything under 25 mIU/L. Functionally, we prefer to see it under 8 — ideally under 5. A fasting insulin of 18 with normal glucose isn’t fine. It’s a signal.

“Can you add fasting insulin to my bloodwork?”

Sex Hormones: Estradiol, Progesterone, Testosterone

Why these three together: Estrogen dominant relative to progesterone — one of the most common patterns in women 35–50 — drives weight gain around the hips and midsection, bloating, and fluid retention. Low testosterone affects lean muscle mass, which directly affects how your body burns and stores energy. You need all three to understand the picture.

Reverse T3 + Full Thyroid Panel

Why it matters for weight: Reverse T3 is the brake pedal of your thyroid. When your body is under chronic stress, it converts active thyroid hormone into Reverse T3 instead — effectively slowing your metabolism as a protective mechanism. Your TSH can look completely normal while this is happening.

GI Map

Why the gut drives weight: Your gut bacteria produce enzymes that regulate how estrogen is metabolized and cleared from your body. When the microbiome is dysregulated, estrogen recirculates instead of clearing — contributing directly to estrogen dominance and the weight patterns that come with it. No amount of dietary change addresses this without knowing what’s actually happening in the gut.

Comprehensive Functional Bloodwork

Weight rarely has one driver. Our comprehensive panel looks at all three sugar markers together, 10+ hormones in relationship to each other, inflammation markers, and thyroid function as a complete picture. The pattern tells a story that no individual marker can.

Bloating, digestive issues, food reactions

Comprehensive Metabolic Panel (CMP)

What it is: A broad panel covering liver function, kidney function, and electrolytes.

Why it matters here: The liver is your primary detox organ — and when it’s under stress, digestion suffers. The CMP is a starting point to rule out liver stress and flag any electrolyte imbalances that affect gut motility.

CBC with Differential

Why it’s relevant here: Elevated eosinophils (a white blood cell type in the differential) can signal food sensitivities or parasitic activity. Most GPs won’t flag this unless it’s extreme — but it’s worth knowing.

Full Thyroid Panel

Why thyroid affects digestion: An underactive thyroid slows gut motility — the speed at which food moves through your digestive system. Constipation, bloating, and a feeling of slow digestion are common thyroid symptoms that rarely get connected to the thyroid.

“I’d like a full thyroid panel — TSH, Free T3, Free T4, Reverse T3, and TPO antibodies.”

Fasting Insulin

Why blood sugar affects the gut: Insulin dysregulation affects the gut microbiome directly — high sugar availability feeds opportunistic bacteria and drives dysbiosis. If your digestion has worsened alongside dietary changes that increased carbohydrate load, this is worth checking.

“Can you add fasting insulin to my bloodwork?”

GI Map (Gastrointestinal Microbial Assay Plus)

What it is: A comprehensive stool test that maps your gut microbiome — the bacteria, parasites, pathogens, and fungi present in your digestive system, along with markers for inflammation and gut immune function.

Why it matters: The GI Map tells us specifically what’s driving your symptoms — so we can address it specifically, not with a generic probiotic or elimination diet. Pathogenic bacteria, low diversity in beneficial strains, fungal overgrowth, parasites — each one points to a different intervention.

What we’re looking for: Pathogenic bacteria, low beneficial diversity, fungal overgrowth (including candida), parasites, markers of intestinal permeability, and calprotectin — an inflammation marker specific to the gut lining.

Mucosal Barrier Assessment

What it is: A test measuring secretory IgA (sIgA) — your gut’s first-line immune defense — along with markers of intestinal permeability.

Why it matters: Your gut lining is one cell layer thick. When it’s compromised — through stress, NSAIDs, antibiotics, or chronic dysbiosis — proteins and bacterial fragments cross into the bloodstream and trigger a systemic inflammatory response. This is the mechanism behind food reactions that seem to be getting worse over time, and sensitivity to foods that never bothered you before.

Low sIgA specifically tells us the gut is under-resourced immunologically — often the result of chronic stress depleting the gut’s defenses over time.

Category 04

Mood changes, poor sleep, cycle irregularity

TSH

Why thyroid affects mood and sleep: Thyroid dysfunction — in both directions — directly affects serotonin production, sleep quality, and emotional regulation. It’s one of the first things to rule out before assuming mood changes are purely psychological.

Vitamin D (25-OH)

Why it matters here: Vitamin D receptors are present throughout the brain. Low Vitamin D is directly linked to low mood, seasonal mood changes, and disrupted sleep architecture — particularly the deep sleep stages.

Sex Hormones: Estradiol, Progesterone, Testosterone

Why these drive mood and sleep: Progesterone is calming and sleep-promoting — it has a direct effect on GABA receptors, the same pathway targeted by sleep medications. When progesterone drops (as it does in perimenopause, often before estrogen does), anxiety, poor sleep, and mood instability follow. Estradiol affects serotonin directly. Testosterone affects motivation and emotional resilience. These three markers together explain a significant portion of mood and sleep changes in women 35–50.

Full Thyroid Panel: Free T3, Free T4, Reverse T3, TPO Antibodies

Why the full panel matters here: Subclinical hypothyroidism — normal TSH, low Free T3 — is one of the most common and most missed drivers of depression, anxiety, and disrupted sleep in women. TPO antibodies can cause fluctuating thyroid output that produces mood swings that look psychological but are hormonal.

DHEA-S

Why adrenal health affects mood: DHEA-S is the raw material for sex hormone production. When adrenal reserve is depleted, the downstream effect on estrogen and testosterone is significant — and that shows up as mood instability, low motivation, and poor stress tolerance.

Saliva Cortisol Panel

Why the timing of cortisol matters: Cortisol and progesterone share a precursor — when your body is under chronic stress, it preferentially makes cortisol over progesterone. This is called “pregnenolone steal.” The result: low progesterone, disrupted sleep, heightened anxiety. A 4-point saliva test maps the cortisol curve across the day and tells us whether this pattern is active.

GI Map

The gut-brain connection: Your gut produces approximately 90% of your body’s serotonin. When the microbiome is dysregulated, neurotransmitter production is directly affected. Mood changes, anxiety, and sleep disruption that don’t respond to standard interventions often have a gut component that hasn’t been investigated.

Brain fog, joint pain, skin issues

Why it matters here: Elevated white blood cell counts and specific patterns in the differential can surface chronic immune activation — a common driver of brain fog and systemic inflammation.

Why it matters: Liver stress and electrolyte imbalances affect cognitive clarity and can contribute to systemic inflammatory patterns that show up in the skin and joints.

Why it matters here: Vitamin D is a potent immune regulator. Deficiency is directly linked to increased inflammatory activity, joint pain, and skin conditions with an inflammatory or autoimmune component.

Why thyroid drives these symptoms: Brain fog is one of the most consistent symptoms of low thyroid function — specifically low Free T3. Dry skin, hair loss, and joint stiffness are also classic hypothyroid symptoms that rarely get attributed to thyroid unless TSH is significantly out of range.

Sex Hormones: Estradiol, Progesterone, Testosterone

Why hormones affect brain function and inflammation: Estradiol is neuroprotective — it supports cognitive clarity, memory, and focus. As estradiol declines in perimenopause, brain fog often follows. Low testosterone affects mental sharpness and motivation. These aren’t just quality of life issues — they’re hormonal and addressable.

Why the gut drives systemic inflammation: When the gut lining is compromised, bacterial fragments and undigested proteins enter the bloodstream and trigger a systemic immune response. This is the mechanism behind joint pain, skin flares, and brain fog that seem unrelated to digestion — they often are gut-driven, just upstream of where the symptoms show up.

Mucosal Barrier Assessment

The permeability connection: Low sIgA and elevated permeability markers tell us the gut’s immune barrier is compromised — and that the inflammatory signals driving your symptoms have a root that hasn’t been addressed.

Comprehensive Functional Bloodwork

Why hs-CRP matters here: hs-CRP (high-sensitivity C-reactive protein) is a sensitive marker of systemic inflammation — far more accurate than standard CRP. Most GPs don’t run it. It’s a central marker in our comprehensive panel and one of the clearest indicators of whether inflammation is actively driving your symptoms.

The Bigger Picture

Why your labs need to be read together — not one at a time.

Here’s what conventional medicine misses: markers don’t operate in isolation. Your body is a system, and what looks “normal” in isolation can tell a very different story in context.

Three examples:

TSH is 2.1 — normal. But Free T3 is low, Reverse T3 is high, and ferritin is 22. This woman has been told her thyroid is fine. What she actually has is a conversion problem her body can’t make active thyroid hormone — and depleted iron stores making it worse. Three markers. None of them flagged alone. Together, a clear picture.

Fasting glucose is 94 — normal. But fasting insulin is 19 and HbA1c is 5.6. This woman is in the early stages of insulin resistance — years before it becomes a clinical diagnosis. This is the window where intervention changes the trajectory entirely.

Estradiol is within range. But progesterone is low and DHEA-S is depleted. Relative estrogen dominance, adrenal exhaustion, and the symptoms that follow — sleep disruption, weight gain, mood instability — none of these appear on a standard panel because no one ran the full picture.
